missing forms icon on pages & posts

    I have wordpress 3.1.2 using the Atahaulpa theme. The Issue is missing ‘add form’ icon for new page & post.

    I have gone to PlugIns – Mailpress – AddOns and activated Forms v5.1.1;
    However, nothing happened – no icon appears.

    I have gone to PlugIns – New – Install WordPress Form Manager v1.3.2;
    However, no icon appears

    what else needs to happen to allow me to incorporate a form on a page or post or at least see the form icon?

    update: I deleted and reinstalled the Form Manager plug in and now I have a settings option on the Form menu, but when I select it i get the following:
    Table ‘gmbaccom_wrd3.wp_fm_settings’ doesn’t exist

    Any assistance would be helpful
