Plugin for verticle site feedback button.

Hello,
Is there any free plugin that shows verticle feedback button on the side of the site like at http://www.diversesolutions.com/ site.
when clicked then through a lightbox a form appear and user can send their suggestions/feedbacks on a specific email.

there are some but they need paid registeration on sites like
http://getsatisfaction.com/partners/wordpress
http://www.simplefeedback.com
and by kampyl
But I need the free one, means totally free.
