• Hello Everyone,

    I am a newbie with using custom forms and NuSOAP so please bear with me.

    I have installed exec-php plugin.
    I have uploaded all of the Custom Form files in to the root directory and also pasted the form.php code in a sidebar widget area.

    All pages are working fine but after clicking the “Submit” button of the form I get the following Fatal error message:

    Warning: require_once(lib/nusoap.php) [function.require-once]: failed to open stream: No such file or directory in /home/xxxx/public_html/xxxxxxxxxxxxxxxx.COM/life-insurance-brain.php on line 4

    Fatal error: require_once() [function.require]: Failed opening required ‘lib/nusoap.php’ (include_path=’.:/usr/lib/php:/usr/local/lib/php’) in /home/xxxxxx/public_html/xxxxxxx.COM/life-insurance-brain.php on line 4

    How do I fix that, PLEASE?

    I have tried using different themes but I get the same problem with each of them. I am using WordPress 3.0.5

    All files of the NuSOAP “Lib” and “Form files” are uploaded in the same /public_html/ document root directory
