When a deletion is refused, Media Cleaner leaves the item in Issues on purpose: it only deletes when it is sure the results are current. It is almost always one of these two things.
- A scan was paused or interrupted. While an unfinished scan is waiting, the results are locked and nothing can be deleted. At the top of the Dashboard you can resume it or discard it. Let it finish (or discard it), then delete again.
- The results come from an older version. Deleting needs a scan completed by the version you have installed, and 7.2.8 came out the day you posted. Run a new scan, keep the page open until it is fully finished, and deleting unlocks by itself.
If you removed the picture from the WordPress Media Library directly, that finished scan will also clear it from the list.
If the item still stays after that, a message appears when you click Delete that says exactly why. Could you paste it here? I will sort it out with you.
Also, let me know if you use any caching/performance plugins, they tend to freeze whatever data comes from the database.
