Thanks for the clear breakdown.
The key point is that your Earned Points (13) are higher than your Total Points (6), which is causing the 217% result. That should not happen, so the score is being calculated incorrectly.
To look into this properly, could you please share a screenshot of the point value set for each question, along with a screenshot of the overall quiz settings that decide the pass or fail. It would also help if you could send the link to a free course on your site that includes this quiz, so we can take it and test the scoring on our end.
Note: One clear solution to try first: open the quiz builder, set each question’s Points to 1, save, and take the quiz again.

I figured out the issue – some points scoring had not been updated. I note that you have to update EACH QUESTION for it to update properly, you can’t just click UPDATE QUIZ, because then the individual questions don’t get updated. Perhaps this could be improved your end as this is not intuitive. I would expect that clicking UPDATE QUIZ would update all changes made at once.
One error still remains however – Answered Questions are 6 and yet Total Questions are only 5. How do I resolve this please? Total Questions should be 6. There are 6 questions.
On submission of quiz, it shows:
Total Questions 5
Answered Questions 6
Quiz settings show:
Full Points 6
Pass Points 5

Thank you for the update, and for sharing what you discovered. You are right that changes made to individual questions need to be saved on each question, and that clicking Update Quiz alone does not save those question-level changes. That is helpful feedback, and I agree it is not very intuitive, so I will pass this on to our team as an improvement suggestion.
For the remaining error, the mismatch you are seeing is a strong clue. When Total Questions shows 5 but Answered Questions shows 6, it usually means one of your 6 questions did not save properly and is still sitting as a draft rather than being fully saved in the quiz. Because of that, the quiz only counts 5 questions as active, while the attempt still recorded an answer to the 6th. This fits exactly with the saving behavior you just found, where a question can be left un-updated.
To fix it, please open the quiz, then open each of the 6 questions one by one and save or update each question individually, making sure none of them is left in a draft state. After that, run a fresh test attempt and check that Total Questions now shows 6.
If it still shows 5 after re-saving every question, could you please share the link to the course or quiz so we can test it on our end and look into it further?

After investigating the quiz issue you mentioned, we found that the quiz you had duplicated was not saved properly on your site due to caching. To fix this, we manually added a new quiz and deleted the duplicate one, which has now corrected the total number of quizzes shown in the stats.
