I’ve analyzed your requests. The first one is the extra request that was intercepted from the visitor while they were filling out the form.

To fix the issue, please add the following exclusion in your plugin settings here:
Anti-Spam by CleanTalk → Advanced settings → category “Exclusions” → Form Signs Exclusions
