Exit Survey upon Delete Account

I would like to know if there’s a way to add an “Exit Survey” for users who choose to delete their account.

I would like the survey to appear either just before they delete their account (preferred) or just after (if it’s the only way).

It could appear when they click “Delete Account” from the Profile page or it could redirect to a new page with the “Delete Account” button also acting as a Submit button for the Survey. Or something to this effect.
