Please open the form in the editor and navigate to Behavior > Rendering. From there, enable the following options:
- Load form using AJAX
- Prevent page caching on form pages
After enabling these settings, kindly check whether the issue is resolved.

Thank you for your response. After conducting additional research, I discovered that Divi 5 will not display the form associated with the shortcode in the visual builder. It does, however, display correctly on the live page. As I understand it, this is by design in Divi 5. Because the form design takes place in Forminator, that is where all styling must take place. Now everything makes perfect sense. The “Edit form” message that appears in the visual builder is instructing us to do exactly that. As a suggestion, changing the message to “Edit this form in Forminator” might help clear up the type of confusion I experience as a new user.
