Ai form doesn’t generate

Hi there

I’m trying to create the following form:

Application form:

Fields for the following:

Two columns for First Name, Last name (next to each other)

Two columns for  Email,  Phone (next to each other)

Two columns with the following:

1. menu with title: Tell me more About?  Drop down the following to choose from :

-EQASA Modules

-Horse Masterships

-Brainline Equine Science

-Stable Yard Management

2. Menu with title: Available Start Date,

Must be able to choose date from Calendar

Below that a message field  where they can type more info

Then a Button below that says apply now

Form submission must go to melise@crystalridgestables.biz

It goes to up to 100% to creat it but it stays there and don’t finish creating the form

I also tried do do a manual form but still not coming right, I also se a Json error apear when I try and publish the manual form

I have reset my permalinks, I have deactivated all the plugins

Please advise
