    Our site has a number of forms like this where we ask people to upload an image file (file types are restricted to JPG and PNG). However, in spite of working well in the past, for some reason most of these forms are not able to be submitted. I have already:

    1. Rebuilt the forms
    2. Tested with plugins turned off to determine conflicts
    3. Tested on a basic theme
    4. Turned AJAX on (and off)
    5. Checked in with my hosting, they cannot detect errors and the uploads directory has read/write permissions set
    6. Tested the forms without uploading a file (and they send just fine)

    At this point, I’m kind of at a loss. Please help!

    I hope you are doing well and sorry to hear you are having this issue.

    I made a submission test and it worked well “Thank you for contacting us, we will be in touch shortly.”, it was a JPG image relatively big but I didn’t face any issue.

    Can you replicate the same problem or are you getting random reports?

    I did see that go through. Thank you!

    The problem happened on a different form, so I’ll do a compare and contrast between the two and see what’s going on.

    So it looks like it was a combination of a bunch of different things. Ultimately, I did need to do all of the above AND redo the old form from scratch. That was the only difference left between the one you tested and the one remaining that wasn’t working.

    Could you please confirm if it works properly now? Also, if you had an issue on the form itself end, maybe something not properly configured or a specific setting was not set, causing this?

    Yes, the form is working now. I’m pretty sure the cause was that it was an older form and somewhere in the update cycle it stopped working. I noticed that some of the settings and options were very slightly different.
