Thanks for reply. Yes I can, but I have never used it so I am not sure what preset to use. If I only fill in the code in the left window I can see that my conditional fields are not showing on the right where the actual form is. So I guess something is wrong? But on the page where the form is live, the conditional fields are showing.

No need to use any presets. You can paste the conditions as well by switching to text mode in the Conditional Fields tab of your form. I’ve just created a video with a brief introduction on how you can use the form tester: https://www.youtube.com/watch?v=Zm_02PD3AXg

I have now filled in all the information, but yes, I receive “Undefined value was submitted through this field” on some fields when I send, and I don´t understand why. You had some duplicate field names in your form. This isn’t allowed. All field names should be unique.
I fixed this by changing the duplicate ogonklinik to ogonklinik and ogonklinik-value. I did the same for ogonsjukdom and ogonoperation.

Now that works, but now I seem to have a problem with the plugin Cookie Notice & Compliance for GDPR / CCPA. That is of course not your problem. When activated, forms will not send. But with your help, my forms are now correct. 🤗
