Disquis Plug in

I have installed disquis and aksmit plugins, intered aksmit key, checked the aksmit box, checked subscribe box, and was hoping to get boxes for name, email, website, comment, subscribe, and trackback. However, as you can see here I am missing some things http://educatorssite.com/

I also added the widget “recent comments” which is not showing up either.

Any suggestions as to how to fix these problems?
