I don’t think that what you’d like to do is currently possible.
Can you share more about your use case? I’m curious to know why you’d like to fill up a title and description file without adding any actual file on it.
Perhaps we have a solution for that we’re not considering at the moment.
I am currently working on clinic system using the Jetpack CRM, so we want to insert note for every session. Sometimes, we do want to insert document for every session but sometimes we just want to insert short note.

We have a feature request open to add the note functionality in Client Portal.
In the meantime, as a workaround, I suggest you add empty files with just a title and a few lines if needed, or check if adding a custom tab can help you. Those suggestions aren’t ideal, but I hope they can help in some capacity.
