Thanks for reaching out to us.
Did you intend on reaching out to WPForms support and not WP Mail SMTP support?
If that is the case, we would recommend either reaching out to them via their support forum (if you are using the free version), or if you are a Pro license holder, then you should instead reach out to WPForms Email support via your WPForms account.
I suspect that there may be a plugin conflict that is affecting WPForms, as a quick test you can try quickly disabling your other plugins in case one of them is conflicting with WPForms.

I don’t know why you think I’m reaching out to the wrong group. I’ve installed WP Mail SMTP plugin – so why would I reach out to WPforms?

Thanks for getting back to us and I apologise for not clarifying.
The issue you described appears to be primarily related to WPForms.
WP Mail SMTP does not have a feature nor a conflict that would cause a WPForm to not display.
As I mentioned in my initial response, this sounds like a plugin or theme conflict, so the fastest means of troubleshooting this issue would be to disable your plugins temporarily (excluding WPForms) to see if there is a particular plugin that is causing the issue.
To directly confirm if the issue is due to WP Mail SMTP, you can try disabling WP Mail SMTP then refreshing the page.
If the issue is still occurring, then the previous step would be recommended. Another possibility is that that particular form has been deleted so the tag has no form to load. You can confirm this by checking your current WPForms and trying to re-insert a new one on that page (or a different page in case the issue is page specific).
