Please enable this option “Enable if Subscribe Form Layout Problem/Input Field Overlap” and check it out. ( Admin Dashboard ->Instock Notifier ->Settings ->Troubleshoot Settings -> Enable if Subscribe Form Layout Problem/Input Field Overlap )
