I’m creating a campaign specific donation form, and all of the customizations I add show up just fine in the preview until I publish. Once published, the form shows up as a completely different form that is in use on another section of the site. Even the shortcode populates the wrong form.

When I accessed your website here https://www.monroehumane.org/donations/best-friend/ a saw a donation form and it loaded well for me. Am I missing something here?
