Unable to complete test transaction

I set up a page and added form short code to make a test transaction. I set up a sandbox in Square and entered values. I’m using free version 4.0.2. I’m using WP 5.9.1.

When I submit the transaction I get the credit card and it stays there permanently. In Chrome Developer Tool Console I get an error:

In the WP Easy Pay –> Square Connect menu I can enter a notifications email and I am showing Test Payment. I also have a notification that Square is connected. The country currency defaults to USD and under location dropdown I’m not given options.
