New Posts Are Strange (images and code)

First, the facts:
I have searched looking for this answer, and have tried the posted solutions. They have not worked. I have posted screenshots on my WP blog for reference to this post: http://www.lancedaoust.com/?p=169

I have access to MacPro, MacBook Pro, iMac and Dell Laptop. The problem is the same on all computers.

Second, the problems:

CODE IN VISUAL VIEW:
As you can see from the screenshot, the code is visible while in the visual view. This should not be the case.

CANNOT SELECT IMAGE SIZES:
As you can see from the screenshot I cannot select thumbnail, medium or large, however full size is defaulted. One of the suggestions was the regenerate thumbnails plugin. I installed it and it didn’t change anything. Another solution was to re-upload WP, which also did not work.

WHAT CHANGED:
I migrated from one linux server operating WHM/cPanel to another server operating WHM/cPanel. cPanel migrated the accounts and data for each account. Everything seems to be completely normal, except for these two things. This is the same on all computers and on all my blogs, which are also on the same server.

THE SOLUTION:
This is unresolved at the time of posting.
