you need huge code customization, with the help of a developer, in order to do that. You have to customize the template but also the rest of the plugin should be modified in order to handle these fields.
But, will it be possible to have help to change the form?
Because for example, I had seen that in the premium plugin we were able to define which contact form we wanted correct?

these are features only for the commercial version of the plugin, I’m sorry.
But with the commercial version I can choose the form fields and which form to choose. If you want you could choose contact form 7 ?
