Know of an Insurance Quote Plugin?

Hi. Trying to build insurance quote forms that allows info to be sent to an email account and simultaneously to a database.

Also, needs to be flexible enough to code in zip code validation for certain areas.

I’ve looked at a few few of the form related plugins but so far nothing that is extensible enough for be to get under the hood and code some extras in.

Does anyone know of a solution to this? Im trying to avoid coding the forms in php fully.
