Could you please just try switching to a default WP theme and then check whether the form loads fine or not? This seems more like either the theme is overriding the form design and causing the issue or it might be more with the website being minified.
If switching to a default WordPress theme resolves the issue then could you please check whether clicking the “Refresh” button as mentioned here helps:

If the above doesn’t help much and still have issues regarding this. Then please do share the form export, so that I could compare how the form behaves in order to see what might be causing it.

Hi, Im joining because I have the same display issues – basically the custom colour settings do not apply at all and am left with a transparent background for the calendar and also for dropdown selection.

It was raised before and at the time the offered CSS solution helped me, so you may try it and see how it goes:

Note: don’t forget to change to your form in CSS or remove form ID number at the end, so CSS will apply to all Forminator forms.

I tried changing the theme, but the problem still exists, as per your suggestion, I have exported my form and the Exported Formlink is given below.

Forminator is a great plugin which allows you to do more and more in free.! So that thing is also free.

On the shared website the issue is related to HTTP and HTTPS.
The website is loading under SSL HTTPS but the styles URLs from HTTP, it makes the browser to block the style request:

Please, go to WordPress > Settings > Make sure the HTTPS link is configured.
You can use a plugin like https://wordpress.org/plugins/really-simple-ssl/ to fix any mixed content.
