I have to say that this is the first time I see a report for this, there isn’t anything that should cause such behavior. We get exports of forms pretty much daily while providing support here but never experienced an issue like that.
Can you give us some more info about how the form was transferred over to your client, did you use Forminator export/import feature or some other method?
Do you still have the form on your end so you could export it and import again on clients site?
Is there any “cleanup” plugin running on your clients site or their server that might’ve caused this?

Hi Predrag,
I should have been more clear – the form was built on my clients site. By handing it over, I simply told the client it was up and where to find it. As far as I can tell, there are no cleanup plugins installed on this site. My client called his host, they mostly shrugged their shoulders saying WP installs are not their problem. Sadly, they only run backups on the 1st of the month, so no hope there.
As he tells it, my client said he simply went to edit a field in the form, pressed save/update, then went to see the page his form was on. That’s when he found the form was no longer appearing. He’s not the most technically proficient, but at the same time I can’t see him manually deleting all fields by accident.
The next morning when I received the report, I logged into his site. In the Forminator dashboard the form title is listed under forms. I can see there are two submissions (although there are all blank now). But if I go to Edit the form, all fields are gone. Email notifications gone, and PayPal integration gone.
The only oddity I can see on the site is that WordPress is complaining the host is running PHP 7.2.34.
Really at a loss as to what happened. Unfortunately, as the form was built on the clients site, we have no backup of it to restore again. Any thoughts?

Thank you for the information.
We suggest using PHp 7.4 which is the version recommended on WordPress.
About the plugin, it is an odd situation, all content is saved in the database, unless it had any problem on the forminator_form_meta table the form would load fine.
I am not sure if the DB will help, as the plugin get the data from there, missing fields could indicate the data is missing in DB too.
You can look at your PREFIX_postmeta table > forminator_form_meta, find the one that matches your form ID in postid column.
