I think the problem is that you haven’t set a product price. In this case WooCommerce hide the add to cart button and you don’t see the add-ons. Try to set the product price to ZERO instead of empty.

I understand your idea and I confirm to you that need a product price.
If you set the product price to ZERO and add the price for each add-ons, and make the add-ons required all works like you want.
However, in general, our plugin use the add to cart button hooks to show and add the add-ons feature. If you don’t set a price WooCommerce hide the add to cart button, the requirement actions for add-ons aren’t available and the plugin stop to works.

maybe do you have customized templates in your theme folders (for the add-ons)?
Please try to activate a default WordPress theme (i.e:Twenty Twenty) and see if you have the same issue.
