widget cannot show normally

I am using woomyreward 4.2
I have already followed all steps in document to set myreward -> feature -> facebook setting.
I also put the related short code into the page.
But I don’t know why the widget cannot show normally?
Is it anything I need to take attention? Do I need to go live for facebook app? Also in part of Install your App on your Facebook Page. I only get public_profile permiss. How can I choose other permissions?
