I am sorry about the issue you are experiencing and I am happy to assist you with this.
W3 Total Cache writes the .htaccess rules in the block. Meaning within the #Begin W3TC and #End W3TC.
So you need to make sure that other rules are placed outside of the W3TC rules to avoid any issues.
You mentioned that the issue started recently, so possibly some rules you added are out of place.
Are there any other rules besides W3TC rules?

Hi and thanks for your reply – the other rules are always outside the W3 block – the plugin is simply overwriting the whole file, having done this at least 5 times in the past 10 days

As impossible as that sounds, can you please share the website URL, the .htaccess file (when the issue occurred), the location of your .htaccess file and are those rules manually added or by some plugins?
