WP plugin for download specific file/content

Hi guys,

I have a specific pdf file that I want to host on my wp site. This file should be available to customers for free download. In order for them to downloaded it for free, they need to fill out a small form – nothing more then a name and email. So, when a customer fills the form (writes his name and email) i need the wp to agregate (to save the customers info) and takes the customer to the download link or to just open the download window instantly.

Is there a plugin that can help me do this?
