double domain name shows up on url

I’m a newbie and have been using wordpress for few months, I have got it running but lately I have installed the plugin phpMyadmin 2.10.3 for backing up. Later I realize my blog did not work, somehow it had created a double name. Below is my Blog:

But the THIS WORK: http://blog.fantasyweddingphotography.com/ but again going to different pages will create double domain name again and not work. I have tried to restore my SQL by using the same program phpMyadmin but it has given me error. Is there a way I can restore everything back? Any help would be appreciated. Thank you.
