Shortcodes Showing in Subject and Body of Message, Data Missing

Hi,

Shortcodes have started showing in the subject and body of messages. Also the email address no longer shows in the body of the message.

For example I get the below:
Subject: [recipient-label] inquiry from website

From: John Smith
Phone Number: 111-111-1111

Interest: [recipient-label]

The mail template is:
Subject
[recipient-label] inquiry from website

Message Body
From: [your-name] <[your-email]>
Phone Number: [your-phone]

Interest: [recipient-label]

Message:
[your-message]

The form is
