    I am having continued issued with the registration page:
    http://headandneckpathology.com/register/
    Even though the form is filled out completely, I cannot complete registration due to an error in the “Membership Type” – Even though I have selected a membership type, I receive an error saying “Please specify account type” and I cannot complete registration.

    Also, I am trying to log in with a known username and password
    http://headandneckpathology.com/login/
    And I receive an error “Too many failed login attempts. Please try again in 56 minutes.” Even though this is the first time I have tried to login.
    Please advise.

    I temporarily deleted the user role from the form, re-instated the user role, and i think it works now.
