    Hi there,

I have a CFF form that worked very well previously but has stopped working:
    1) It no longer presents the data that has been retrieved from a new wordpress table (wp-distances) via MySQL into the form field which also uses Google map APIs.
    2) Weirdly, I have found that if I insert the form shortcode into the page twice, the first one doesn’t work and the second one does !
    3) On the first form entered on the page it neither displays the retrieved table data nor does the calculation.
    4) on the second form, it does not display the retrieved data either but does execute the calculation and display the result (so it is obviously correctly retrieving the table data but not showing it).

I am seeing the same behaviour on two different pages:
    http://www.save-the-planet.org/staging/testing
    http://www.save-the-planet.org/staging/offset-my-flight

I am using Avada theme, WooCommerce and CFF developer edition.
I have tried all combinations of script load method, character encoding, activating JS cache, form cache on the form with no success.

    Thank you.

The page I need help with: [log in to see the link]
