MaxBlogPress Opt In Form

Hi~ Newbie back for more help! I installed a plugin for an opt in form. The one by MaxBlogPress. I created the html automatically in my IContact account. Then I copied and pasted it where I was directed to. Next I was instructed to click on “Detect Form”. Did that and now I have no idea where to go with the “Name Field” and “Email Field”, and the “Tracking Code Field”. I went to the support forum for that plugin and registered, as they tell you to do that and paste your contact form html code there in a question so they can tell you how to proceed. That was yesterday. I am still waiting for the confirmation email to arrive, until then I can’t post a question in THEIR help forum! Can you help me on this?
