What is the version number of your copy of the plugin?
Be sure the plugin has been activated properly in the different websites.
Actually is very difficult identify what is happening exactly on your website, because I don’t know the way you have defined the multi-site. One of my website is a multi-site installation of WordPress, and it is working with no problems.

Plugin is currently running version 1.0.334.
Plugin has not been network activated but activated on each individual sub site.
The multi-site is using sub directories rather than sub domains. Zero configurations have been made to the network from the WordPress defaults.

Seems like it was as simple as updating and network activating the plugin. Once I did this the Sample forms appeared on the backend. I then added the form and it finally appeared in the form list.

If you watch my video, I’m not activating the plugin at network level, it was activated only in one of websites.
