Form requires Login

All;
You will see on the blog page a link to the Fall tagging form.

I have created this simple form in the free version of VFB and when the public browses to the page and selects to open the form, the page requests a login.

I suspect that There is a setting that I need to modify but I am unable to find the correct setting.
