cleaning database

There is a lot of bad content in my database, inserted by spam bots or whatever, even if they do not affect the appearance of my site..

what might they be doing?

and how can i clean it? most efficiently? manually i go in there and remove bad stuff by hand one by one? is there a shortcut? how?
