Customer Orders Always Fail

Hello,
I am editing the back end of a client’s webpage, mysteriousmeats.com, and we have run into some problems. So far, every time a customer has tried to order an item off of our website the order has failed.

Every failed order (which is every order so far) gets this error message:

Order status changed from Pending payment to Failed.Specifically, this is what a failed order looks like: https://www.dropbox.com/s/juhx0zluq8exggu/Failed%20Order%20ED.jpg?dl=0 - Note the error message under "Order Notes" on the right. ALL the failed orders have this error message.
How do I fix this problem?
I'm not sure if it's related, but the shipping calculator also is inconsistent. I will post another topic in this forum earlier about the shipping calculator issue.
