First please enter plugins\form-maker\js and replace this file main_div_front_end.js with the one that you can download from here.

Then please enter plugins\form-maker\frontend\views\form_maker.php and delete the below script from the line 1579.
' . $disabled_submit . '

We have checked the form and it seems you haven’t done this part of the recommended fix.

Then please enter plugins\form-maker\frontend\views\form_maker.php and delete the below script from the line 1579.
