WordPress not displaying form tables right?

Hi, at the bottom of this page http://manmeetswoman.com/about-man-meets-woman/special-talk-july-29th-dating-and-relationships-for-the-modern-man/

I set up my table/form code to look wider, with the text fields wider as well. WordPress didn’t seem to like the code and kept showing this, no matter what.

I really had a nightmare with trying to use checkboxes and radio buttons. The code/html is from aWeber and I customized it a little in dreamweaver before inserting.

Now, I’m not a CSS or code expert, but I’d like to know what I did wrong and how I can have more control (and show radio buttons that look aligned properly, lol) in the future.

Can anyone help?
