Missing Attendee Fields

Hello,

I’m using using the Event Manager Pro plugin and I need to add the ability for users to add additional attendees and there names to the form registration. I’ve come across the documentation found here: http://wp-events-plugin.com/documentation/how-to-add-an-attendee-form/ but additional name fields are not populating.

1. On my event>forms editor page, I’ve enabled ‘attendee form’ and created the default form.
2. On my event page I’ve selected the form from the ‘Attendee Form’ section
3. I’ve updated the ‘Maximum spaces per booking’ to 5

On the front-end when I select more than 1 space from the drop down, the additional attendee form fields do not appear.

Is there any way to solve this?
