Configuration Errors – But nothing changed on form

I did validate the parameters are correct and nothing has changed recently, other than plugin updates. Emails still come in but not in the format shown in the screenshot. Can anyone provide guidance on the issue I am seeing?

Here’s to hoping a 3rd party plugin didn’t break CF7.
