Activities attendance

Hello. We need a database to register children attendance to each activity in a non profit community center. Is it possible to have something like the following?
1. A Students register form (with general info of each student)
2. An Activities register form (with general info of each activity like date, title, category)
3. An Attendance register that by selecting a student name and an activity, combines the student´s info and the activity info in one record?
