We would like some more information.
1. Can you confirm you are using the most recent Yoast SEO, v9.5?
2. Are all your non-Yoast plugins and themes also updated?
3. Can you provide a link to the site in question?
I think I have seen the same on http://nutruehealth.com/ Google Search Console says Sitemap can be read, but has errors, Unsupported file format. Using 9.5, all up to date.
@one3rdnerd I see weird code before each page:

It’s possible that your website is hacked. You can see it when you open page source of any page. Also, it could break many things (sitemaps, redirects, login, … ).

As the sitemap is being outputted correctly I can only suggest the following two actions:
1. Clear all your caching from your theme, plugin, server, CDN like CloudFlare or browser. If you are not sure how to clear caching from theme/plugin please speak to those authors. If you want to clear cache from the server, please speak to your host provider. To clear the cache from a browser use this guide: https://kb.yoast.com/kb/how-to-clear-my-browsers-cache/.
2. Delete your sitemap_index.xml file from your Google Search Console. Doing so will not hurt the SEO of your site.
3. Add the sitemap using this guide: https://kb.yoast.com/kb/submit-sitemap-search-engines/ and see if the issue resolves.

So after checking and stumping my host and seo experts..after about a week it just submitted fine. Odd
