Help Identifying Plugin or Solution on this Page

    I have a client who has a landing page that she wants a mock questionnaire like the one on this page with just Yes/No buttons that proceed to the next question whichever is shown and ends with a result that contains an opt-in and link button.

    I cannot seem to find a plugin out there at all that has just the basic Yes/No buttons on a quiz or multi-step form, that would proceed to the next question, I got close with HD QuiZ but not all the way there since I am unable to use HTML or shortcodes in the result to include the opt-in or button. Plus it reloads the WHOLE page just to proceed to the next question…

    Can anyone point me towards a plugin that has these types of functions other than Thrive obviously, I don’t want to pay 67 bucks for such a small time thing…

    If I was doing this it would be with javascript, possibly with AJAX putting the responses into my database.

    I guess it depends upon how much your time is worth to yourself and your client, $67 is not much in the scheme of things.
