It seems like you are using a Registration Type form where username and password fields are mandatory.
If you don’t want these fields you may use a Contact/Other Type form.

When you click on Add New it shows a pop-up to enter the name along with the type of form you want. Choose “Contact/Other Form” and click on Add.
