Email Capture

I am looking to offer a free ebook on my blog. I have a page dedicated to pushing it. I want people to enter a name and valid email in order to gain access to the ebook. I will probably be using aweber (or some other auto responder service) to confirm the email and send out the ebook. Unfortunately I have never done anything like this before and don’t know the best way to go about it…should I build a form or is there a plugin that might help?
