“nome” e “ultimo nome” desappear

Hello
we have a problem with our site. in order completion, when customers have to fill in their data, the “name” and “last name” fields do not appear and they are required fields. This does not allow orders to be placed on our site. These fields disappeared for no reason.
What can I do to resolve it?
