CUSTOM FORMS

I need desperate help on this…

I’m trying to set a custom table/form in my wordpress site…

This the page I’m trying to build:

This form/table gets data from an existing Joomla site located at this url:
http://www.fondmetal.com/index.php?option=com_content&view=article&id=217&Itemid=377&_f=ricerca&lang=it

PROBLEM, nothing works on WP…when you try and select a field the page simply reloads…
Can anyone help me out with this ?
