   Hi there, can anybody tell me if there is a simple widget or a way to create something like an application form, say for instance I want to make a custom application form for people to fill in on the website (electronically) and with a hit of a button to send back to my email address? Please help!!! Thanks

   Hi vtxyzzy, thanks this was also what I was looking for but tell me, isn't there a plugin like where I can perhaps make something like a pricelist where people can check certain items and send it back to me as an order? Thanks for your help

   It is more comples to set up, but TDO Mini Forms might do what you want.
