Custom page template doesn’t stick – reverts to default page template

I have created a custom page template for the homepage on my site. Randomly it has started reverting back to the default page template. I have created dozens of sites with custom templates and have never run into this issue before. This is a HUGE problem for my client. Anyone know what to do?
