Just a short text entry field, with maybe some optional text (like &#8220;Please let us know why you are reporting this comment&#8221;). That text could also be an option in settings.<\/p>\n<p>Then maybe adding an optional message to pop up when someone reports a comment (like &#8220;thanks&#8230;we&#8217;ll take a look&#8221;.
