It was working great until I changed the permalink to post name, then internal sever error. I think the headers, meaning the wordpress .htaccess created by php and the .htaccess this plugin creates conflicts. If you use default, resulting in no .htaccess file being created on the server and leaving everything up to wordpress php .htaccess methods it works fine.
Maybe I messed something up, but this crashed my site. As soon as i deleted the .htaccess file it created and wiped the plugin, it was back.
This should really all be implemented in the WP-core. Great work!!

I wanted to remove the /author/ baseline and the option was there!
